MAIL INSTRUCTIONS

1) Complete the digital form below (form can not be saved while in progress), and follow the mailing instructions. After completing the digital form you will receive an email with directions on how to pay the application processing fee of $50.

2) Assemble all necessary documents (resume, recommendations and essay).

3) Package the resume, essay, and recommendations (in separate envelopes within the package)

In your application packet, please include a copy of your full creative resume.
Please type an essay (approx. 400-1000 words) telling us more about yourself, include a brief
biography and your artist statement or philosophy. Also, please address one or more of the
following questions in order for your essay to be complete.

• In what way do you envision yourself serving clients, community or culture?
• Who are your heroes? Are there creative entrepreneurs that inspire you?
• Why do you think you would be a good fit for this program?
